Rehabilitation, Helping You Reach Your Goals

A comprehensive, integrated inpatient rehabilitation program makes use of the “team approach” for your benefit. Your treatment team will carefully consider your needs and capabilities, and then — along with your input — develop an individualized plan of care.

Our program places emphasis on:

  • providing the highest level of therapy, medical care, and nursing care;
  • helping you regain independence by teaching you new ways to do ‘self-supportive’ tasks such as laundry, cooking, and cleaning;
  • teaching you the ‘activities of daily living’ (or ADLs) — such as dressing, grooming, bathing, etc. — to help you resume your normal quality of life;
  • transfer training and mobility;
  • assisting with communication skills;
  • providing patient and family education, counseling, and training;
  • dealing with the psychosocial issues that frequently occur after illness or injury; and
  • developing an appropriate, ongoing plan of care that will help keep you well, even after you’re discharged.

You Make the Difference

Though your team of caregivers will support you throughout your recovery, the progress you make depends largely on you. Your hard work and participation are crucial to your ultimate success.

Most of our services are available seven days a week. We participate with Medicare, Medicaid, and most other private and group insurance companies.

Three Rivers Health Inpatient Rehabilitation Offers Solutions

At Three Rivers Health, the Rehabilitation Pavilion is located within the hospital which provides comprehensive acute inpatient rehabilitation. The Rehabilitation Pavilion opened its doors in 1990 and is well known in the Southwest Michigan area for excellence in rehabilitation.

The acute inpatient rehab unit is designed for patients who have experienced health concerns such as traumatic injuries, brain injury, neurological disorders, spinal cord injuries, stroke, amputation, hip fractures and other orthopedic disorders, or other debilitating conditions. These patients require continued physician oversight and therapy after their acute hospital stay — at that point they are stable to transfer but not yet safe or well enough to return home. Patients at inpatient rehab participate in therapies at least three hours per day, with the goal of the patient being able to maximize their recovery so they can return home safely at their highest levels of independence.

The team of professionals at Three Rivers Health includes physical therapists, occupational therapists, speech therapists, nursing, social work/discharge planning, nutritionists and a medical director. This team meets at least weekly to provide a coordinated individualized care plan for each patient. The inpatient rehab unit is HFAP (Healthcare Facilities Accreditation Plan) accredited.

Family members, relatives, and friends of the patient are encouraged to visit with their loved one, participate in therapy sessions for education, and be involved with the treatment planning process. Pets may come to the unit to see their loved one (with appropriate vaccinations). A garden club comes in once a month and a comfort dog visits patients one to two times per month.

Talk to your doctor to see if you or your loved one qualifies to come to an inpatient rehab facility for rehabilitation.
