We would like to thank you for taking the time to learn more about Three Rivers Health Rehab Pavilion. We offer comprehensive inpatient rehabilitation services. Using an interdisciplinary team approach, we customize each patient’s care and treatment. Our goal is to provide comprehensive and continuing care to persons with impairments. Our treatment team focuses on patients’ capabilities, rather than disabilities. This positive approach allows our rehabilitation professionals to help patients return to their highest levels of independence. We help patients with a wide variety of diagnoses and conditions, such as:

Our inpatient medical rehabilitation program at the Rehab Pavilion is fully accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).     Rehabilitation, Helping You Reach Your Goals
       
    A comprehensive, integrated inpatient rehabilitation program makes use of the "team approach" for your benefit. Your treatment team will carefully consider your needs and capabilities, and then — along with your input — develop an individualized plan of care.
       
    Our program places emphasis on:





  •  


    • providing the highest level of therapy, medical, and nursing care;


  •  


    • helping you regain independence by teaching you new ways to do ‘self-supportive’ tasks such as laundry, cooking, and cleaning;


  •  


    • teaching you the ‘activities of daily living’ (or ADLs) — such as dressing, grooming, bathing, etc. — to help you resume your normal quality of life;


  •  


    • assisting with communication skills;


  •  


    • providing patient and family education, counseling, and training;


  •  


    • dealing with the psychosocial issues that frequently occur after illness or injury; and


  •  


    • developing an appropriate, ongoing plan of care that will help keep you well, even after you’re discharged.

    Most of our services are available seven days a week. We participate with Medicare, Medicaid, and most other private and group insurance companies.
