|  | Donation can be made by check and should be mailed to Three Rivers Health Foundation, 701 S. Health Parkway, Three Rivers, MI 49093. Donations can also be made in person at Three Rivers Health in the main lobby area. Please ask them to contact the Foundation Director at extension 300.
